LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide licensed nursing care to residents in long-term care or skilled nursing facility settings. Support patient well-being through direct care and collaboration with healthcare teams to ensure quality outcomes.

Responsibilities:

• Administer resident medications and treatments accurately and timely.
• Monitor and record patient vital signs, symptoms, and responses to treatments.
• Assist with activities of daily living, including bathing, dressing, and ambulation.
• Collaborate with healthcare professionals to develop and implement care plans.
• Ensure compliance with safety protocols and facility policies.
• Document patient care meticulously in medical records.

Qualifications:

• Valid LVN or LPN license to practice in the state.
• Proven experience in long-term care or skilled nursing environments.
• Excellent communication and interpersonal skills.
• Ability to perform clinical assessments and prioritize patient needs.
• Knowledge of infection control procedures and safety protocols.
• Ability to work flexible hours, including weekends and holidays.
