Registered Nurse

Care Scout is seeking a compassionate Registered Nurse to guide families through complex long-term care decisions for aging loved ones. Responsibilities

• Conduct comprehensive nursing assessments of older adults' health and functional status
• Develop and refine personalized long-term care recommendations for families

• Collaborate with care advocates and provider networks to coordinate services
• Review clinical records, labs, and provider notes to inform care guidance
• Monitor care outcomes and adjust recommendations as needs evolve
• Document assessments, care plans, and family interactions accurately in EHR/CRM systems
• Ensure recommendations align with safety standards and regulatory requirements
• Participate in cross-functional meetings to improve care navigation tools and processes
• Contribute clinical insight to content, resources, and decision-support tools for families
