LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide skilled nursing care to residents in a Long-Term Care / Skilled Nursing Facility setting. Administer treatments, monitor patient progress, and collaborate with healthcare teams to ensure quality resident care.

Responsibilities:

• Assess residents' health status and document clinical observations.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and respond to changes in condition promptly.
• Collaborate with interdisciplinary teams to develop and update care plans.
• Maintain accurate and timely documentation of care activities.
• Adhere to all safety and infection control protocols.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience working in LTC or SNF environments preferred.
• Demonstrate knowledge of medication administration and clinical procedures.
• Exhibit strong communication and interpersonal skills.
• Ability to assess patient needs and respond effectively.
• Maintain current CPR and first aid certifications.
