LVN​/LPN – LTC​/SNF

Job Summary:

Provides skilled nursing care to residents in long-term care or skilled nursing facilities, ensuring the delivery of high-quality patient care in accordance with established protocols and regulations.

Responsibilities:

• Administer medications, treatments, and procedures as prescribed by physicians.
• Monitor, record, and report on residents' health status and response to treatments.
• Assist with activities of daily living, including bathing, dressing, and mobility assistance.
• Maintain accurate and detailed resident records in compliance with regulatory standards.
• Collaborate with interdisciplinary teams to develop and update care plans.
• Ensure infection control protocols and safety procedures are consistently followed.

Qualifications:

• Current LVN/LPN license in good standing.
• Valid state or federal issued identification required.
• Proven experience providing nursing care in LTC/SNF settings preferred.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
• Must have completed the COVID-19 vaccination or have a valid exemption.
