Southwest Care Center (SCC) is a non‑profit FQHC located in Santa Fe and Albuquerque, New Mexico. We are looking for a Case Manager to join our team of professionals who serve patients in our clinics. The role requires a strong desire to make a difference in the community, a team‑oriented approach, and the ability to lead and mentor medical support staff.
Responsibilities- Collaborate and coordinate with health‑care team members to promote interdisciplinary treatment planning.
- Conduct intake interviews and perform psychosocial assessments to identify psychosocial, economic, or social risk factors; provide eligibility and referral services to patients and families to support adherence to medical treatment plans.
- Assess and maintain eligibility in health‑insurance plans, patient‑assistance programs, or other services that improve retention in care; coordinate assistance by making referrals, following up with patients and agencies, and monitoring progress through the service‑delivery system.
- Provide consultation regarding financial planning, budget development, and housing plans.
- Link patients to a broad range of services including medical care, behavioral health, substance‑abuse treatment, financial assistance, housing, food, clothing, transportation, vocational training, and harm‑reduction services.
- Monitor and accurately document patient progress through individualized service planning that assesses level of functioning, adherence to medical treatment plans, and access to service needs using the electronic medical record (EMR).
- Within scope of practice, provide therapeutic interventions to patients by ensuring adherence to intervention plans, engaging patients through education and supportive activities, monitoring medication regimens, follow‑up appointments, and utilization of enabling and supportive services.
- Perform prior authorizations, collaborating with clinical staff and providers for necessary documentation.
- Maintain knowledge of applicable grants, program funds, and regulatory requirements (such as FQHC, PCMH); ensure patient counseling, guidance, and documentation are consistent with those requirements.
- Develop and maintain strong relationships with community resources to stay up‑to‑date on available services.
- Assist with outreach HIV/HCV rapid‑testing and counseling programs (following certification).
- Strong desire to make a difference in the community.
- Comfortable leading and working with a team of medical support professionals.
- Ability to mentor others and participate in quality and process‑improvement initiatives.
- Excellent communication and interpersonal skills.
- Knowledge of behavioral health and community resources is preferred.
