Registered Nurse

Job Summary:

The Registered Nurse will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and other team members is critical.

- Provide skilled nursing care.

Required Skills &

Certifications:

- Active Registered Nurse license in the state of working.
- Clinical Assessment, Medication Administration, and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and automobile for field work.
- Minimum of 2 years of hospital, home health, or hospice care experience.
- Ortho and stroke experience.
- AHA BLS Provider CPR.
