Job Overview
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient‑centric care management services for at‑risk patients in an acute care environment. Accountabilities include assessment and planning, care coordination, resource utilization management, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. The goal is to ensure appropriate care is delivered at the right time and setting.
Responsibilities- Assess and plan patient care for at‑risk patients
- Coordinate care across the continuum
- Manage and review resource utilization
- Plan and facilitate discharge
- Document interventions and maintain regulatory compliance
- Advocate for patient needs
- Associate's Degree in Nursing (degree or diploma) at hire
- California Registered Nurse License at hire
- At least 2 years Acute Care experience in a Medical Surgical or Inpatient setting
- Experience in care management or utilization review (any setting)
- Successful completion of TIPS program or Case Management Orientation Program at hire
- Bachelor's Degree in Nursing or higher within 3 years of hire
- Case management certification (ACM or CCM)
Pay range: $57.28 - $88.92 per hour. Additional compensation may be available for shift differentials, overtime, etc. The successful candidate's placement on the wage range will be determined by relevant job experience and other applicable factors.
