We are seeking an experienced Registered Nurse – Case Manager to join an acute care hospital’s Case Management team. The RN Case Manager will be responsible for coordinating patient care, performing admission reviews, and managing discharge planning while ensuring efficient utilization of healthcare resources.

The ideal candidate will have strong experience in care coordination, utilization review, and discharge planning within an acute care hospital setting.

• Perform patient assessments and care coordination throughout hospitalization

• Conduct admission reviews using Inter Qual criteria

• Develop and manage individualized plans of care

• Coordinate discharge planning and post-acute care services

• Arrange durable medical equipment (DME) and required patient services

• Collaborate with physicians, nurses, and social services to optimize patient outcomes

• Review benefits eligibility, prior authorizations, and admission criteria

• Maintain accurate documentation using Epic EMR

• Case Management / Utilization Review

• Admission Criteria Review (Inter Qual)

• Care Coordination

• Discharge Planning

• Needs Assessment and DME Ordering

• Prior Authorizations

• Plan of Care Management

• Experience working with HMOs, IPAs, and insurance authorizations

• Patient Population: Adults and Geriatrics

• Caseload: Up to 20–22 patients per day

• Patient Ratio: Approximately 1:20

• Interdisciplinary Support: Social Services

• First-time travelers are not accepted

• Candidates must have recent acute care hospital case management experience

• Maximum 7 days RTO allowed

• One pre-approved time-off request allowed at submission

Required Qualifications

• Active RN License (California)

• Minimum 2 years of Acute Care Hospital Inpatient Case Management experience

• Current experience within the last 6 months

• Strong knowledge of Inter Qual criteria

• Epic EMR experience

• Experience with Care Coordination and Admission Reviews

• Travel experience required (seasoned traveler preferred)
