Licensed Vocational Nurse (LVN)
Under the supervision of a licensed healthcare professional, and with a narrow latitude for independent judgment and decision, this position works closely with the treatment team to administer care to the participants in a 24/7 residential and recovery facility. The LVN helps to ensure clients are safely monitored throughout their residential stay and all facets of the day-to-day operations are run efficiently.
Essential Duties and Responsibilities- Patient Care
- Provide nursing care to patients in a residential treatment setting, including monitoring vital signs, assessing withdrawal symptoms, and reporting changes in patient condition.
- Support oversight of participant day-to-day operations and services.
- Conduct timely and comprehensive bed checks according to schedule and in accordance with program guidelines.
- Ensure clients are safely monitored throughout their residential stay.
- Assist management team and treatment team with client supervision and service delivery.
- Testing and Documentation
- Ensure regular drug screens are conducted as directed by management, ensuring that tests are logged appropriately.
- Complete incident reports as required.
- Assist in maintenance of program records and files.
- Maintain accurate and timely documentation of patient care in electronic health records (EMR).
- Provide clear and concise shift reports to ensure continuity of care.
- Safety and Compliance
- The LVN must adhere to all state and federal regulations, including California Board of Vocational Nursing standards and prioritize patient safety and dignity in all aspects of care.
- Adhere to all safety rules and regulations.
- HIPPA Compliance:
Secures participant information and maintains strict participant confidence by completing and safeguarding medical records and ensuring participant information is kept confidential. - Professional Behavior:
Strictly maintain professional boundaries following established limits and guidelines for appropriate behavior with participants, ensuring a safe and ethical environment by clearly separating the professional role from personal interactions, and protecting the client's privacy and dignity. Boundaries typically include but are not limited to personal disclosure limits, physical touch restrictions, and clear communication about the professional nature of the relationship. - Ensure health and safety regulations are upheld to CLARE|MATRIX standards, OSHA, CAL-OSHA, and all regulatory requirements.
- Medication Administration
- Inventory Management:
May monitor medication stock levels, medication inventory, organize medication storage, and report low stock or discrepancies to supervisors. - Prepare, administer, and document all medications according to physician orders and facility policies.
- Monitor patients for potential side effects or adverse reactions to medications.
- Educate patients on purpose, potential side effects, and proper use of prescribed medications.
- Billing for Medication Monitoring
- All notes must be completed in real time, or at least by the end of shift to expedite billing.
- Submit UA results in MyEvovle to assist in billing process.
- Ensure health and physicals and TB test administration are reflected in the charts for billing to process timely and accurately.
- Teamwork and Collaboration
- Communication:
Collaborate with nurses, therapists, and other healthcare providers to communicate participant changes, needs, updates, and progress. Respond to staff and participants in a professional and courteous manner. - Collaborate with cross-functional, and multidisciplinary teams including, but not limited to Department Heads, Counselors, therapists, case managers, and medical professionals to ensure comprehensive participant care.
- Participate in staff meetings, one-on-one meetings with supervisors, and other meetings as required.
- Maintain knowledge, understanding and application of CLARE|MATRIX's mission, vision, and philosophy.
- Education and Training:
Participate in ongoing training, team meetings, and certifications to maintain eligibility for the role.
- Other duties as assigned
- Must be a graduate of an accredited LVN program.
- Must have a current and unrestricted California LVN license.
- Must have a minimum of 1-2 years of experience in a residential or substance abuse treatment setting is preferred.
- Experience in a residential or recovery center setting preferred.
- Knowledge of residential treatment protocols and withdrawal management for alcohol and substance abuse.
- Strong assessment and clinical skills with the ability to respond quickly to changes in patient condition.
- Excellent communication, organizational, and time-management skills.
- Effective verbal and written communication and interpersonal skills.
- Organized and detail oriented.
- Self-motivated and able to effectively and accurately follow directions and carry out job duties and responsibilities.
