Providence Saint John’s Health Center – Santa Monica, CA. Part‑time 8‑hour day shift RN Care Manager responsible for professional, comprehensive, patient‑centric care management services for at‑risk patients in an acute care environment.
ResponsibilitiesThe Inpatient Registered Nurse (RN) Care Manager will provide professional, comprehensive, patient‑centric care management services for at‑risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.
Qualifications- Associate’s Degree in Nursing upon hire
- California Registered Nurse License upon hire
- 2 years Acute Care experience in a Medical Surgical or Inpatient setting
- 1 year Experience in care management or utilization review in any setting, OR
- Upon hire successful completion of TIPS program or Case Management Orientation Program
- Bachelor’s Degree in Nursing or higher within 3 years of hire
- Case management certification (ACM or CCM)

Pay Range$57.28 – $88.92 per hour
