The Long-Term Acute Care (LTAC) Registered Nurse provides specialized, ongoing care for medically complex adult patients. Operating within a hospital-affiliated LTAC unit in Columbia, MO, this role centers on continuous patient monitoring, complex clinical interventions, multi-system management, and collaborative discharge planning. You will work alongside an interdisciplinary care team—including physicians, therapists, and case managers—to observe subtle clinical trends, manage high-acuity needs, and optimize patient recovery.
Key ResponsibilitiesPatient Assessment & Care: Perform detailed clinical assessments, monitor vital signs and lab trends, administer ordered medications, manage complex IV therapy/infusions, and provide specialized wound, dressing, and ostomy care.
Complication Management: Vigilantly monitor for acute clinical changes or complications; elevate findings promptly to attending physicians according to facility protocols.

Collaboration:
Work closely with physicians, therapists, case managers, and family members to build, evaluate, and adjust individualized, comprehensive care plans.Documentation &
Education:
Maintain precise, timely clinical documentation in the Electronic Health Record (EHR). Educate patients and caregivers on treatment plans, medication management, and post-discharge expectations.
Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program.
Licensure: Active, unencumbered Registered Nurse (RN) license in the state of Missouri (or a valid Multi-State/Compact RN License).
Certifications:
Basic Life Support (BLS) for Healthcare Providers (Required)
Advanced Cardiovascular Life Support (ACLS) (Recommended)
Specialized LTAC, Progressive Care (PCCN), or Critical Care (CCRN) certification (Preferred)
Experience &
Skills:
Demonstrable experience with complex inpatient acute or post-acute care, telemetry, wound management, and IV therapy. Strong clinical judgment, interdisciplinary communication, and EHR documentation skills are essential.
Bachelor of Science in Nursing (BSN).
Hands-on experience with mechanical ventilation and tracheostomy care.
Proficiency with advanced care coordination platforms and hospital EHR systems.
Hospital-based LTAC environment with rotating shifts, including evening, night, and weekend coverage as dictated by unit needs.
The work environment is steady with moments of sudden high acuity during acute patient changes.
Prioritizes patient safety, collaborative team dynamics, and structured discharge planning.
