Job Description

Summary:

With the support of and in partnership with the LEAD team, Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals, and is responsible for identifying, engaging and providing support to homeless clients referred from the LAPD Pacific division, including aftercare services and follow up support, once housed. The goal of LEAD is to connect with clients experiencing frequent contacts with police due to their homeless status and enroll them in services to prevent entry to the jail and criminal justice system.

The Case Manager provides information and support for program members, and assists members in linking to a variety of resources and services as needed. The Senior Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.

Job Description

Essential Duties and Responsibilities:
  • Identify, engage, and provide support to clients in the LEAD program living on the streets in Pacific Division, targeting those clients who are vulnerable due to medical, psychiatric, or addiction needs.
  • Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition
  • Work collaboratively as a team to identify appropriate services, taking into account each individual’s physical and cognitive needs
  • Provide case management and aftercare/follow-up services to clients placed in permanent housing
  • Report all statistics and updates for reporting purposes
  • Provide initial contact, needs assessment, information, education and referrals to the homeless
  • Provide case management, basic services, and advocacy for clients
  • Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.)
  • Develop effective, trusting relationships with clients that facilitate provision of services
  • Work in collaboration with LEAD ODR team members and outside agency staff to connect clients to appropriate services
  • Maintain and record accurate electronic records, client files, and statistical information
  • Attend all staff meetings, trainings and retreats
  • Participate in OWG and other community meetings as assigned.

Qualifications:
  • High School Diploma or GED or Equivalent
  • Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
  • Willing and able to transport Clients in your personal vehicle.
  • Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)
  • Willingness to work in a high tolerance Harm Reduction model with clients who have multiple barriers
  • Working knowledge of LA County agencies serving the homeless
  • Excellent human relations/communication skills; ability to act in professional manner regardless of circumstances
  • Skilled in non-violent crisis intervention
  • Ability to self-motivate and organize time effectively and efficiently
  • Computer literate; able to use computerized database system for information management
  • Passion for working with the population served
  • Ability to work as part of an interdisciplinary team

Preferred Qualifications
  • Bachelor’s degree; or a minimum two years’ experience working with homeless individuals, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience.
  • Combination of field and office environment.
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites.
  • Will necessitate working in busy and loud environments.
  • Will be exposed to elements like cold, heat, dust, noise and odor.
  • May need to bend, stoop, twist, and sit throughout the day.
