Position:
Central Intake and Triage Nurse (Hospice) – Collective bargaining position.
Schedule:
5x8 hour shifts (5 days per week, full-time 40 hours).
The Hospice Central Intake and Triage Department is the main access point for all Providence Hospice customers, including current patients, family members, physicians, physician office staff, inquiries, and others. The Central Intake and Triage Nurse may arrange prescribed services with the physician, payor, patient/caregiver, and provider to ensure timely and coordinated services. This position will also process triage calls from existing patients/caregivers, address clinical issues when appropriate, and forward to the appropriate team supervisor as necessary.

- Establish and maintain positive working relationships with all Hospice Services customers.
- Prioritize multiple tasks and work in a demanding, fast-paced setting.
Patient Population Served:
Adult, Geriatric. The employee must interpret a patient s self-report or behavior with understanding of cognitive, physical, emotional/psychosocial, and developmental factors. Treatment should be individualized to reflect developmental needs and range of treatment needs for each patient.
- Represent the mission of Providence Health and Services in all central intake and coordination functions.
- Take referral information online and access necessary information (history and physicals, inpatient notes/summaries, labs, physician names/addresses) to complete referrals.
- Coordinate with other Providence Home Services departments if additional services are required.
- Identify patients most likely to require hospice and schedule an intake or assessment visit.
- Work with the Hospice Insurance Coordinator to investigate and document insurance eligibility, benefits, and required authorizations for all referrals.
- Communicate information to patients/caregivers, physicians, and providers; assist with arranging alternatives based on insurance coverage and benefits.
- At time of referral, obtain complete insurance information online (name of insurance, insured person, relationship to patient, group/, employer, and verification contacts).
- Collaborate with hospital and extended care facility care managers, discharge planners, physician offices, and other referral sources to plan home services for patient discharges in the local and regional service area.
- Maintain accurate daily referral and on-call logs by referral and insurance source for monthly statistics.
- Establish and maintain positive relationships with home care providers, payors/case managers, physicians and staff, and Providence Home Services departments.
- Maintain a customer-service focus at all times.
- Attend scheduled staff meetings twice per month and participate in department quality improvement initiatives and inservice education.
- Performs other duties as assigned.
- Preferred – Bachelor s Degree in Nursing or Social Work; degree in related field may be considered.
- Preferred – Call Center experience.

- Required upon hire:
Oregon Registered Nurse License (Vendor Managed) or Washington Registered Nurse License (Vendor Managed). - Required upon hire: RN License in the state of hire and wherever care is delivered.
