Formerly Aurora Santa Rosa Hospital, SRBHH opened in 2016 and is regarded as an integral part of the greater Santa Rosa and Northern California mental health system of care. Conveniently located in the North Bay, this hospital consists of 144 acute psychiatric inpatient beds and serves adolescents and adults, treating both mental health and co‑occurring psychiatric and substance abuse conditions.

DESCRIPTION OF POSITION

The top priority of the Assessment and Referrals RN is to perform accurate assessment of incoming admissions in order to deliver high quality safe care during the Intake process. The RN comprehensively processes patients from receipt of referral until transfer to unit, assessing needs via crisis calls, walks, or referral packets to ensure the appropriate level of care per EMTALA guidelines.

The RN utilizes facility policies, exclusionary criteria, and capabilities to accept patients for treatment, obtains MD orders, and determines appropriate unit assignment. The RN embraces SRBHH’s mission, vision, and values and adheres to all hospital policies and procedures, ensuring compliance with regulations, standards, and requirements. The RN maintains knowledge, participates in continued training, and understands occupational health hazards such as blood borne pathogens.

KEY RESPONSIBILITIES
  • Respond to telephone and face‑to‑face inquiries from persons seeking mental health care or information about services.
  • Interact with patients, significant others, and appropriate providers to collect and analyze data on physical, psychosocial, environmental, self‑care, educational, spiritual, cultural, and anticipated discharge needs; determine alcohol or drug toxicity; identify signs of withdrawal; and conduct screenings or assessments to determine treatment needs.
  • Deliver competent and timely care based on current chemical dependency and psychiatric nursing/clinical knowledge and standards; evaluate care against goals and plan and revise as needed.
  • Facilitate the patient’s access to the appropriate level of care and setting.
  • Establish and maintain effective working relationships with patient services counselors and coordinators, intake department, physicians, health professional affiliates, and other nursing staff.
  • Assess client crisis and clinical indicators via phone and in‑person, document on a call sheet, and schedule assessment as clinically appropriate.
  • Prepare written report of assessment results immediately following completed assessment, including recommendations for level of care and treatment needs.
  • Make appropriate interventions during assessments and crisis situations with clients, family/friends and other professional staff.
  • Assess need for involuntary hospitalization evaluation and contact county crisis team, law enforcement or 5150 Certified employees as necessary to ensure safety.
  • Enter call and intake assessment data into the computer system in a timely manner.
  • Assist clients not admitted to a program with follow‑through of clinical referrals and recommendations.
  • Provide pertinent clinical information to physician and program staff reviewing the patient to ensure a smooth transition.
  • Distribute documentation of admissions to the Business Office, Admitting Unit and the Utilization Review Department.
  • Maintain knowledge of developmental tasks and sociological theories of pertinent age groups.
  • Maintain knowledge of legal guidelines pertaining to EMTALA/COBRA and abuse/neglect reporting.
  • Communicate with managed care organizations to obtain insurance pre‑authorizations promptly.
  • Stay up to date with the organization’s programs and services.
  • Participate in activities which enhance professional growth and development.
  • Perform other related duties as assigned.
  • Uphold the organization’s ethics and customer service standards.
  • Cross‑train as Inpatient Registered Nurse and perform duties such as assessing inpatients, providing direct care, documenting patient care, administering medications, and ensuring end‑of‑shift documentation and treatment plan completion.
