Full Time Registered Nurse Case Manager
Territory:
Santa Rosa/Sonoma
Registered Nurse Home Health Case Manager positions require in‑field patient visits in patients’ homes. The role focuses on creating, implementing, and executing comprehensive plans of care to promote wellness and prevent rehospitalization.
Responsibilities- Perform full assessments of patients and their families during initial visits.
- Use assessment data to develop advanced plans of care, including rehabilitation components.
- Work one‑on‑one with patients and families to achieve improvement and stability.
- Case‑manage patient care episodes in collaboration with interdisciplinary clinical teams.
- Prepare clinical notes, orders, and charting within compliant time frames.
- Collaborate with physicians and agency staff to deliver optimal care.
- Initiate and administer IV medications (antibiotics and diuretics) in accordance with established protocols.
- Concierge support to relieve administrative tasks.
- Flexible work schedules; create your own hours.
- Small, manageable territories.
- Autonomy with no clocking in/out.
- Multiple advancement tiers for RNs, including admission, RNI, RNII, and leadership opportunities.
- Low turnover and high morale.

- Current, unrestricted CA RN license.
- Current BLS/CPR certification.
- Valid CA driver’s license.
- Strong clinical knowledge in home health care (highly preferred).
- Experience with high‑acuity patients (a plus).
- Experience working with interdisciplinary teams.
- Case‑management fundamentals.
- Experience with IV medication infusions (highly preferred).
- Proficiency in initiating intravenous therapy (a plus).
- Self‑directed, empathetic, critical thinker.
- Outstanding interpersonal and leadership skills.
- Highly organized.
