PRN Licensed Practical Nurse

Snap Care is seeking a PRN Licensed Practical Nurse to provide high-quality patient care in facilities around Santa Rosa, CA. In this flexible, per-diem role, you will deliver direct bedside care, administer medications, perform treatments, and accurately document patient status in accordance with care plans and facility policies. You'll collaborate with RNs, physicians, and interdisciplinary teams to support optimal outcomes and a safe environment.

Responsibilities

• Provide direct patient care in accordance with LPN scope of practice and facility protocols
• Administer medications and treatments accurately and safely
• Monitor, document, and report changes in patient condition to the RN or provider
• Assist with admission, transfer, and discharge processes
• Collaborate with nurses, physicians, and interdisciplinary teams to support care plans
• Maintain accurate, timely electronic and written documentation
• Adhere to infection control, safety, and regulatory standards
• Communicate clearly and compassionately with patients and families
