Licensed Practical / Vocational Nurse – LPN/LVN
Position OverviewThe Licensed Practical Nurse (LPN/LVN) provides skilled nursing care in private duty settings under the supervision of a Registered Nurse. Duties include delivering care, documenting activities, monitoring patient condition, and coordinating with the care team while maintaining confidentiality and regulatory adherence.

Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and report changes to supervisor or physician.
- Document all activities, assessments, nursing actions, and care coordination in a timely manner.
- Implement, update, and participate in nursing care plans.
- Initiate appropriate nursing actions based on assessment and achieve desired outcomes.
- Recognize changes in patient needs and intervene to prevent or reduce risk.
- Comply with Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
- Apply infection control measures per OSHA standards.
- Educate patient and family on disease process, self-care, and prevention strategies.
- Maintain knowledge of nursing competencies through continuing education and skill evaluations.
- Graduate of an accredited school of nursing.
- Active, unrestricted state license (LPN/LVN) in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies.
- Reliable transportation.
- Current TB skin test within last 12 months.
- Six (6) months of recent experience as a Licensed Nurse in a clinical setting.
- Home health experience.
- Speak, read, write, and understand English.
- Travel willingness.
- Lift 50 pounds and reposition patients without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Enter various environments, possibly with exposure to allergens, blood, bodily fluids, or infectious diseases.
- Respond physically and mentally to emergency situations.
- South Carolina – 1 year pediatrics experience.
- California – 1 year experience under current nursing license.
- Louisiana – 1 year experience as licensed nurse.
- Continuing education as required by state.
