Licensed Practical / Vocational Nurse – LPN/LVN

The Licensed Practical Nurse (LPN/LVN) provides skilled nursing care in private duty settings under the supervision of a Registered Nurse. Duties include delivering care, documenting activities, monitoring patient condition, and coordinating with the care team while maintaining confidentiality and regulatory adherence.

• Deliver and coordinate quality patient care in compliance with physician orders.
• Continuously observe and assess patient condition and report changes to supervisor or physician.
• Document all activities, assessments, nursing actions, and care coordination in a timely manner.
• Implement, update, and participate in nursing care plans.
• Initiate appropriate nursing actions based on assessment and achieve desired outcomes.
• Recognize changes in patient needs and intervene to prevent or reduce risk.
• Comply with Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
• Apply infection control measures per OSHA standards.
• Educate patient and family on disease process, self-care, and prevention strategies.
• Maintain knowledge of nursing competencies through continuing education and skill evaluations.

• Graduate of an accredited school of nursing.
• Active, unrestricted state license (LPN/LVN) in the state of practice.
• Current CPR certification.
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies.
• Reliable transportation.
• Current TB skin test within last 12 months.

• Six (6) months of recent experience as a Licensed Nurse in a clinical setting.
• Home health experience.

• Speak, read, write, and understand English.
• Travel willingness.
• Lift 50 pounds and reposition patients without assistance.
• Prolonged walking, standing, bending, kneeling, reaching, twisting.
• Enter various environments, possibly with exposure to allergens, blood, bodily fluids, or infectious diseases.
• Respond physically and mentally to emergency situations.

• South Carolina – 1 year pediatrics experience.
• California – 1 year experience under current nursing license.
• Louisiana – 1 year experience as licensed nurse.
• Continuing education as required by state.
