In Home Healthcare LVN: Low Acuity

Position Overview

Licensed Practical Nurse (LPN/LVN) – Days and Nights, Full Time Availability. We are seeking compassionate nurses to provide skilled care to a special client/patient in a home setting located in Pflugerville, TX.

Monday through Thursday: 7 AM to 7 PM
Night shifts available throughout the week

• Active, unrestricted LPN/LVN license in the state of practice.
• Compact licenses transferred to state of residence within 90 days.
• Current CPR certification with hands‑on component (training available post‑hire).
• Tuberculosis skin test current within last 12 months.
• Reliable transportation.
• High school diploma or equivalent; graduate of an accredited nursing school.
• Evidence of clinical assessment and documentation proficiency.
• State‑specific experience if required:
  South Carolina – 1 year pediatrics;
  California – 1 year nursing practice;
  Louisiana – 1 year nursing practice.
• Preferred:
  Six months of recent clinical nursing experience; home health experience.

• Deliver and coordinate quality patient care in compliance with physician orders.
• Continuously observe and assess patient condition, reporting changes to supervisor and/or physician.
• Document all activities, assessments, nursing actions, and care coordination in a timely manner.
• Participate in nursing care plan implementation and updates.
• Take appropriate nursing actions based on assessments to achieve expected outcomes.
• Recognize changes in patient needs and intervene promptly to prevent or reduce risk.
• Adhere to Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
• Apply infection control measures in accordance with OSHA standards.
• Educate patient and family about disease process, self‑care techniques, and prevention strategies.
• Maintain professional competencies through continuing education and skill evaluations.

• Speak, read, write, and understand English.
• Travel to patient homes.
• Lift up to 50 pounds and reposition patients without assistance.
• Engage in prolonged walking, standing, bending, kneeling, reaching, twisting, and clinging stairs.
• Maintain visual, hearing, smell, and fine motor acuity.
• Respond mentally and physically to emergency situations in the home or during transport.

• Work in varied environments, potentially exposed to allergens and various conditions.
• Possible exposure to blood, bodily fluids, and infectious diseases.
