Qualifications
  • Active, unencumbered license (LPN/LVN) in the state of practice
  • Compact licenses transferred to state of residency within 90 days
  • Current CPR certification (hands‑on component) – Aveanna can assist with obtaining this after hire
  • TB skin test current within the last 12 months
  • Reliable transportation
  • Six months prior hands‑on nursing experience preferred but not required
Position Overview

The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, following the care plan and physician orders for each patient while maintaining confidentiality and professional boundaries.

Essential Job Functions
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe, assess patient condition and care needs, and report changes to supervisor or physician.
  • Document all activities, assessments, nursing actions, responses, and coordination of care promptly.
  • Participate in, implement, and update nursing care plans.
  • Take appropriate nursing action based on assessment to achieve expected outcomes.
  • Recognize changes in patient needs and intervene to prevent or reduce risk.
  • Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Provide care using infection control measures per OSHA standards.
  • Educate patient and family about disease processes, self‑care techniques, and prevention strategies.
  • Engage in continued education and professional development activities.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Proficiency in clinical assessments, documentation, and compliance with nursing care protocols.
Additional State Specific Requirements
  • South Carolina – One (1) year of pediatrics experience.
  • California – One (1) year of experience working under a current nursing license.
  • Louisiana – One (1) year of experience working as a licensed nurse.
  • Continuing education as required by state.
Preferences
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.

Physical Requirements
  • Speak, write, read, and understand English.
  • Travel to patient homes.
  • Lift up to 50 pounds.

  • Engage in prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Walk and climb stairs comfortably.
  • Maintain visual and hearing acuity.
  • Responsive to emergency situations in the home or during transport.
Environment
  • Functional in diverse environments with potential exposure to allergens and various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
