Skilled Home Health Registered Nurse

Job Description

We are seeking a Skilled Home Health Registered Nurse to provide patient care in a home environment. This role requires experience in home health care, including OASIS training, IV skills, and wound care management.

Responsibilities

• Conduct comprehensive assessments of patients' health status.
• Administer medications and treatments as prescribed.
• Perform wound care and management for various conditions.
• Provide post-operative care and manage chronic health conditions such as heart failure and COPD.
• Utilize medical equipment and perform specific procedures, including venipuncture and Picc line care.
• Maintain accurate patient records and documentation per OASIS standards.
• Self-schedule visits in alignment with the needs of the Home Care office.

Required Experience / Certifications / Licensure

• 1 year of skilled home care experience is required (private duty nursing experience alone is not acceptable).
• Valid RN license for the state of practice or compact license.
• Current CPR/BLS certification.
• Recent patient care experience within the last three months.
• COVID-19 vaccination or approved exemption form.
• Flu shot or declination, with mandatory mask-wearing during work hours.

Schedule

This is a day shift position, operating on a 5x8-hour schedule from 08:00 to 17:00, with variations possible based on Home Care office needs. Alternating weekend shifts may be required.
