Adoptive Family Support Specialist

This position will provide referrals and supportive services to adoptive families residing in Circuit 12 and to caregivers who have become permanent guardians of children through the Safe Children Coalition. Utilizing a wrap-around approach, this position will track and monitor the family's progress, aid in the coordination of behavioral health and community services, and act as a family support to help stabilize youth in the least restrictive levels of care.

May involve being on call (telephone) some evenings and weekends (rotating schedule).

Key Responsibilities:
  • Provide comprehensive support to families utilizing the wrap around model as a guide
  • Provide referrals and connections to community providers for families as requested/ needed
  • Help families to identify their strengths and natural supports in an effort to reduce recidivism
  • Monitor behavioral health needs for children and families and coordinate services/ community outreach as needed
  • Assist families with crisis and safety planning as needed
  • Work closely with the family's identified team of supports in an effort to stabilize the family
  • Facilitate family team meetings at least every 90 days with the family and their support teams
  • Assist families with the process/ referrals for higher levels of care and/or APD (Agency for Persons with Disabilities) applications as needed
  • Track the progress of and help to coordinate step down services for children in higher levels of care
  • Participate in community staffings intended to stabilize placement and prevent children from coming into the child welfare system
  • Identify and build positive relationships with community providers
  • Complete field visits with families as required
  • Document work into statewide computer system
  • Routinely meet with supervisor to review plans, issues, and tasks
  • Attend staff development training for professional growth
  • Monitor email on a daily basis. Circulate pertinent information to unit staff members and collaborative community partners as appropriate
  • Other related duties may be assigned and/or special projects may be requested

Starting at $48,000.00

Requirements

Bachelor's degree in the area of psychology, social work, or related human service field or a minimum of three years' experience working with troubled youth and their families and/or mental health services, including administrative responsibility for compliance.
