The Advanced Practice Provider (APP) serves as an integral member of the Cardiology care team, providing high-quality, patient-centered cardiovascular care exclusively in the outpatient setting. Working collaboratively with cardiologists, nurses, patient access specialists, clinical staff, imaging services, and interdisciplinary teams, the APP helps deliver comprehensive cardiovascular care while improving patient access, continuity, and clinical outcomes.
This role offers a high degree of clinical responsibility and professional autonomy within a supportive, physician-led, team-based environment focused on excellence in ambulatory cardiovascular care.
Primary Responsibilities- Obtain, document, and update comprehensive medical histories and perform physical examinations.
- Evaluate, assess, diagnose, and develop evidence-based treatment plans for patients with acute and chronic cardiovascular conditions in the outpatient setting
- Independently manage established cardiology patients while collaborating with supervising cardiologists as appropriate
- Order, interpret, and manage diagnostic testing, medications, and therapeutic interventions within scope of practice.
- Monitor patient progress, reassess clinical status, and modify treatment plans based on clinical response and current evidence-based guidelines.
- Provide comprehensive patient and family education regarding diagnoses, medications, lifestyle modifications, cardiovascular risk reduction, and preventive care.
- Perform timely post-hospital follow-up visits to support smooth transitions of care and reduce avoidable readmissions.
- Improve patient access by providing timely appointments for urgent referrals, follow-up visits, and same-day or rapid-access evaluations as operational needs require.
- Coordinate care with physicians, nursing staff, imaging departments, primary care providers, specialists, and community resources to support optimal patient outcomes.
- Supervise cardiac stress testing and Cardiac PET testing in collaboration with the cardiology physicians and imaging staff.
- Review diagnostic testing and communicate recommendations to patients and referring providers.
- Maintain timely, accurate, and complete medical documentation within the electronic medical record.
- Participate in departmental meetings, quality improvement initiatives, performance improvement activities, and ongoing clinical education.
- Serve as a clinical resource and mentor to nursing and support staff when appropriate.
- Maintain all required licensure, certifications, credentialing, and continuing education requirements.
This is a full-time outpatient position providing direct patient care in Saratoga Hospital Medical Group Cardiology practices. Work schedules are developed collaboratively with department and practice leadership based on operational and patient care needs.

This position is designed as an ambulatory practice role and does not include routine inpatient rounding, hospital consult responsibilities, admissions, or discharge management.
Routine weekend hospital coverage and overnight call are not required for this position. Participation in occasional practice initiatives, educational events, or operational support activities may be requested based on departmental needs.
While this position does not participate in routine overnight or weekend call coverage, the APP may be assigned to provide telephone coverage for the cardiology answering service during designated transition periods, typically the first 30 minutes of the business day and the final 30 minutes before office closure. This coverage supports continuity of patient care while physicians transition to and from overnight call responsibilities.
The APP is expected to triage urgent outpatient clinical concerns, provide guidance within scope of practice, and collaborate with the on-call cardiologist when appropriate.
