Home Infusion

CVS Health seeks a Home Infusion professional to deliver high quality, compassionate care to patients receiving infusion therapies in their homes. In this role, you will coordinate with physicians, pharmacists, and care teams to manage patient assessments, therapy education, and safe administration of medications and supplies. You'll monitor clinical outcomes, ensure regulatory and safety compliance, and support accurate documentation. Responsibilities

• Provide in-home IV and infusion therapy according to prescriber orders and CVS Health clinical protocols
• Perform comprehensive patient assessments and monitor responses to infusion therapies
• Initiate and maintain peripheral and central venous access devices, including troubleshooting and line care
• Set up, operate, and troubleshoot infusion pumps and related equipment in the home setting
• Educate patients and caregivers on medication administration, infection prevention, and home infusion safety
• Coordinate care and communicate with physicians, pharmacists, case managers, and other healthcare professionals
• Support smooth transitions of care from hospital or clinic to home, including scheduling and service coordination
• Document all care, assessments, interventions, and patient education accurately in electronic health records
• Adhere to all regulatory, safety, and infection control standards for home infusion practice
• Participate in quality improvement initiatives, training, and ongoing professional development within CVS Health
