Responsibilities:- Assess the social and emotional needs of residents and develop individualized care plans to meet those needs
- Provide counseling, guidance, and support to residents and their families to address concerns and improve overall well-being
- Coordinate with interdisciplinary teams to ensure resident-centered care and promote positive outcomes
- Facilitate group activities and therapies to enhance socialization and engagement
- Maintain accurate records and documentation of resident interactions and care plans
- Collaborate with community resources and outside agencies to access additional support services for residents
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
- Commitment to providing high-quality care and services to residents
