Bridgewell - - Responsibilities:
Provide support and assist individuals with activities of daily living (grooming, bathing, oral hygiene, feeding, toileting, transferring, ambulation);
Observe residents for changes in condition or behavior and report to the Program Director;
Administer medications according to MAP regulations;
Maintain daily and monthly progress notes according to DDS and applicable standards;
Write annual assessments on individual goals as outlined in the ISP and participate in ISP meetings and other relevant meetings
