You will act as the primary case manager for 3-6 adults with severe mental illness, coordinating medical care and medications, and delivering health education in clients’ homes. The role requires an associates degree in nursing and an active LPN/RN license, with on-site schedule Monday–Friday, 8am–4pm, plus rotating evenings, weekends, and on-call coverage.
