The RN Float is responsible for assisting with the provision of care and services to clients receiving Assisted Living services, Assisted Living with memory care services, or services through the Good Shepherd Comprehensive Home Care license. Provides nursing leadership, direction, and supervisory oversight to licensed/unlicensed staff. Will be engaged in client pre-screening/admission process, developing a person-centered plan of care, and ongoing client assessments.
Routinely engaged in ensuring plan of care for each client is up to date, processed, and communicated timely. Coordinates services ensuring coordination of care for each client and meets continued stay criteria. Provide direction to staff on engaged communication to clients with memory loss. Actively engaged in QAPI initiatives, develops best practices, assists with staff education, and ongoing policy development.
Assists area RNs in developing and updating a person-centered plan of care for each client after assessing/reassessing clients’ medical, nursing, and psycho-social needs by:
- Conducts client pre-screenings and admissions based on current continue stay criteria.
- Develops a person-centered plan of care based on assessment and client/responsible party wants/needs.
- Coordinates/modifies plan of care/orders as needed with physician, as defined by policy.
- Directs, supervises, and teaches staff on services noted in the plan of care.
- Provides nursing services according to an up to date plan of care.
- Consults with physician for approval/modification of plan/orders, per policy as needed.
- Assists in coordinating additional community referral services as needed.
- Generates and monitors an accurate clinical record and physician updates, as defined by policy.
- Engages families/agencies as changes in condition occur that may impact continued stay criteria.
In conjunction with area RNs, monitors and directs all aspects of medication administration and/or self-administration.
- Reviews and monitors all medication of clients and directs the proper administration.
- Ensures medications and order changes are properly processed, documented, properly signed by medical professional, ordered, delivered, etc as per physician and plan of care.
- Ensure medical/nursing equipment and materials working properly for needed interventions/treatment.
- Administers medications/injections, as may be needed/ordered, as defined by policy.
- Monitors medications and medication regime, as defined by policy, and contacts prescribing professional as needed.
- Monitors scheduled/all medication and follow drug diversion and awareness process.
Demonstrates a working knowledge of current nursing practices when performing observation, nursing assessments, clinical updates, post-fall follow-ups, and all mandated processes per regulation.
Demonstrates the ability to routinely assess and assimilate client needs and follow-up with appropriate nursing interventions, timely documentation, and staff training.
Coordinates and directs the provision of delegated nursing services to nursing/unlicensed staff in compliance with current licensure.
Directs and provides nursing supervision of licensed/unlicensed staff involved in the provision of Assisted Living Home Care services.
Will assist/engage in staff supervisory visits, evaluations, coaching, and disciplinary matters as needed.
Maintains ongoing awareness of state mandated and client services training needs.
Will be engaged in on-going training processes based on client needs, policy, and regulations.
