** Registered Nurse (RN) Case Manager Responsibilities
*** Oversee and manage an assigned nursing home household with a strong focus on person-centered care
* Guide and support household staff in meeting the physical, emotional, and psychosocial needs of residents
* Monitors adherence to Standards of Nursing Practice and facility policy and procedures governing the administration of medications; promoting the safe and accurate delivery of medications/treatments in accordance with the physician’s order.
* Administer medications and treatments in accordance with physician orders and facility policies
* Coordinate resident admissions, discharges, and transfers to and from the household
* Admits, discharges, and transfers community members to and from the household.
* Conducts assessments when community members are admitted or are returning from the hospital and on-going for all sections of the MDS assuring compliance to the requirements.
* Complete the Resident Assessment Instrument (RAI), including MDS assessments, CAAs, triggers, and individualized care plans for assigned residents
* Initiates the care plan, states problems clearly with measurable goals and ensures that approaches are reflective of the current status of the resident.
* Monitors timely planning, scheduling, and revising of the RAI process
* Perform nursing skills requiring RN licensure, including IV therapy and blood draws
* Completes case management assessment on all new residents as per regulatory requirements.
* Process, transcribe, and implement physician orders in compliance with established policies and procedures
* Maintain current knowledge of emergency protocols and respond effectively to emergent situations

** Registered Nurse (RN) Case Manager Qualifications
*** Current Registered Nurse (RN) license to practice in the state of Minnesota
* Current Basic Life Support (BLS) certification, including CPR and AED
* Minimum of two years of nursing experience; long-term care experience preferred
* Demonstrated leadership ability to develop, motivate, and guide staff; strong critical-thinking and problem-solving skills; ability to prioritize responsibilities, remain flexible, and perform effectively in high-stress situations
* Excellent verbal and written communication skills in English; ability to work independently, exercise sound judgment, and collaborate effectively as part of a team
* Proven management skills, including planning and oversight of resources and the design and improvement of work processes
