LVN/LPN - Long Term Care

Assignment: 3 Months Contract

Shift: 12 hrs. Nights

Job description:

  • Primary function is to administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care.
  • The LPN/LVN works under the supervision of a Registered Nurse (RN) and coordinates care with the interdisciplinary team as well as the patient/family.
  • The LPN/LVN will deliver care in compliance with State, Federal and Medicare standards.

Responsibilities:

  • Administers prescribed medications and treatments in accordance with approved nursing techniques and facility policies and procedures.
  • Prepares equipment and aids physician during treatments and examinations of residents.
  • Observes residents, records significant conditions and reactions, and notifies supervisor, physician and family of resident’s condition and reaction to drugs, treatments, and significant incidents.
  • Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of resident.
  • Assigns duties and coordinates nursing services on the resident unit to ensure resident needs are met in accordance with instructions of Physician, federal and state regulations and facility procedures.
  • Consults with Shift Supervisor on nursing problems and interpretation of facility policies to insure resident needs are met.
  • Report all complaints or unusual problems to supervisor. Assist with completion of facility complaint report.
  • Participates in orientation and training and evaluating of nursing personnel.
  • Evaluates nursing activities to ensure resident care, staff relations, and efficiency of service.
  • Assigns duties and coordinates nursing service on the resident unit.
  • Observes nursing care and visits residents to ensure that nursing care is carried out as directed, and treatment administered in accordance with Physician's instructions.
  • Assumes responsibility and maintains records for unit narcotics.
  • Inspects resident rooms and the unit for cleanliness and comfort.
  • Responsible to document admissions, discharges, transfers and incident reports per facility policies and procedures.
  • Participate in the care plan process, completes nursing assessments and resident care plans accurately and timely on assigned residents.
  • Completes special projects and audits at the direction of the Supervisor, CCC, or Director of Nursing.
  • Maintain all educational requirements for the State of Michigan to keep license current. Attend facility in services and ward conferences as assigned.
  • Maintain resident confidentiality; treat residents with kindness, dignity and respect; know and comply with all Resident's Rights rules; and promptly report all resident accidents and injuries to charge nurse.
  • Must have regular and attendance.
  • All other duties as assigned.
