Summary
Under direct supervision of the provider, the LPN will conduct patient assessment, coordinate care, and maintain accurate documentation. The role involves providing compassionate patient care, managing supplies, ensuring compliance with My Michigan Health policies and applicable regulations, and training new staff as needed.
Responsibilities- 30% Triaging calls, scheduling urgent/add‑on appointments, relaying test results, supporting consults and pre‑certification.
- 30% Greeting patients, performing vital signs, recording objective and subjective data, medication reconciliation, entering refills within scope, maintaining accurate patient records.
- 20% Assisting the provider with examinations and procedures.
- 20% Preparing and cleaning the patient area, setting up equipment, sterilizing instruments, disposing contaminated supplies.
- Complete patient assessment within scope of practice.
- Perform patient care measures including injections, dressing changes, obtaining blood specimens; may assist with EKG, PFT, ABI, NST as required.
- Assist in maintaining a clean, safe environment; may travel to satellite clinics.
- Operate computer systems: MAPS, MCIR, EMR, LIS, Emageon, dictation; demonstrate critical thinking for patient management.
- Maintain confidentiality and HIPAA compliance.
- Perform clerical duties such as filling insurance forms, correspondence, hospital admission coordination, answering the telephone, filing records; order supplies and medications.
- Ensure accurate documentation and coding; participate in annual quality reviews.
- Provide age‑specific care according to standards and policies.
- Demonstrate competency in Microsoft Windows; may participate in additional learning opportunities.
- BLS – Basic Life Support mandatory.
- LPN – Licensed Practical Nurse, state of Michigan required.
- Technical Certificate or equivalent LPN program required.
- Knowledge of nursing techniques normally acquired through completion of an LPN program.
- Experience:
3 to 5 years in health‑care or related setting preferred. - Annual OSHA training required per Exposure Control Plan.
- Fingerprinting required.
- Working knowledge of Infor Lawson, EMR, and practice‑management systems preferred.
- PC software proficiency:
Excel, Word, Windows. - Excellent interpersonal and communication skills.
- Exposure to stressful situations, trauma, grief, and death.
- Ability to use PPE, including latex when required.
- Mobility across facility; able to lift 11‑20 lbs as needed.
- Vision and hearing sufficient with or without assistive devices.
- Sit, stand, and walk for extended periods; frequent postural changes such as stooping, kneeling, crouching.
- Exposure to blood borne pathogens; must complete required testing.
- Multi‑tasking ability, independent work, reliable attendance.
- Dexterity for handling, reaching, grasping, and repetitive tasks.
- Light physical demand; occasional lift or carry 11‑20 lbs. (0‑33% of the workday).
