Job Summary And Qualifications
The Intermediate/Critical Care Pharmacist provides pharmaceutical care including computer order entry, drug distribution, evaluation of medication orders for appropriateness of drug, dose, frequency and adequacy of monitoring, evaluation of drug interactions and allergies and therapeutic monitoring of drugs to patients across our Stepdown, Intermediate Care and ICU floors.
What you will do in this role:
- You will be responsible for coordinating drug distribution activities in areas under their supervision. Includes supervision of pharmacy technicians, order verification of medication orders, resolution of medication related problems and preparation of parenteral products (IV admixtures, cytotoxic chemotherapeutic
- You will perform pharmacy clinical activities including but not limited to providing pharmacokinetic consults, monitoring targeted drugs, antimicrobial therapy recommendations, and evaluating appropriateness of drug therapy based on, disease state, host factors, and cost effectiveness.
- Serve as a source of drug information to medical and nursing staffs.
- Perform patient care unit inspections as required to meet regulatory requirements.
- Work to minimize errors in the medication use process by utilizing S.T.A.R. (Stop, Think, Act and Review) and other error prevention techniques, especially when dealing with a situation that is out of policy, procedure, or plan, or any other high-risk situation.
