Overview

The Advanced Practice Provider (APP) treats adult patients in a primary care setting, with an emphasis on chronic disease management. The APP performs medical services within the scope of practice for the PA/APRN. The APP collaborates with other team members within the community health center as well as with outside specialty providers.

Education
  • Master's of Science in Nursing or Master’s of Science in Physician Assistant - Required
Experience
  • 2-3 Years - Preferred
License & Certification
  • Professional License with State of Practice (ANPRN or PA) - Required
  • Nurse Practitioner or Physician Assistant national certification (AANPCP, ANCC, or NCCPA) - Required
  • Basic Life Support (BLS) - Required
Core Job Functions
  • Obtains patient histories and conducts pertinent physical exams. Orders necessary testing which could include labs, diagnostic imaging, cancer screenings, and EKGs. Interprets results and informs patient of findings in a timely manner. Diagnoses medical problems and prescribes treatment as medically indicated. Prioritizes shared-decision making and patients’ individual preferences into treatment plan.
  • Utilizes evidence-based and value-based care guidelines and internal procedures to guide diagnostic and treatment decisions.
  • Educates patients with regard to medical conditions, treatment plans, preventive medicine and health promotion topics.
  • Refers patients to specialists when necessary. Follows up with recommendations from specialty referrals. Co-manages patients with specialty providers when indicated.
  • Utilizes available clinical resources. Discusses complicated or challenging cases with the supervising physician.
  • Participates in department quality improvement activities, staff educational programs, attends departmental meetings, and demonstrates clinical competency in the delivery of patient care services.
