Responsibilities- Provides persons, families, or vulnerable populations with the psychosocial support needed to cope with chronic, acute, or terminal illnesses, physical or developmental disabilities.
- Provides patient education and counseling, advises caregivers, and makes necessary referrals for other social services.
- Acts as client advocate to coordinate required services or resolve crises.
- May administer and coordinate services and activities of a specialized program.
- Counsels clients in individual and groups sessions to help them overcome dependencies, recover from illness, and adjust to life.
- Monitors, evaluates, and records client progress according to measurable goals described in a treatment or care plan.
- Refers client or family to community resources to assist in recovery and provides access to services such as financial assistance.
- Coordinate services of various inter/intra community resources across disciplines; serve as a liaison; facilitate family support groups; ensure confidentiality; identify training needs; maintain case files; evaluate staff; complete employee evaluations.
- Entry qualifications include a Master's Degree with license and three years of postgraduate related experience, with at least one year in a supervisory role OR Master's Degree and three years experience at the lower level or position equivalent (HCP
122). - Knowledge of agency and federal policies, procedures, and guidelines; diagnostic impressions or criteria; client advocacy systems; crisis intervention techniques; individual and group therapy techniques; knowledge of various therapeutic models and interventions; knowledge of medications and their effects; knowledge of DSM criteria; knowledge of treatment approaches and psychosocial assessments; confidentiality policies; culturally sensitive interviewing techniques; and related program regulatory knowledge.
- Ability to perform needs assessments, develop and implement treatment or behavioral plans, establish goals, guide policy development, oversee compliance, and coordinate services across resources; ability to communicate effectively with diverse populations; ability to supervise staff and complete employee evaluations.
- Physical:
