As a Home Health Caregiver with Modani Care, you will have the opportunity to make a meaningful difference in the lives of our clients. Your primary responsibility will be to provide personalized care and support to bed-bound patients in the comfort of their own homes. Your expertise and dedication will enable our clients to maintain their independence, dignity, and quality of life.

• Provide direct patient care, including assistance with daily living activities, mobility, and positioning
• Develop and implement personalized care plans in collaboration with clients, families, and healthcare professionals
• Monitor and report client progress, identifying opportunities for improvement and adjustments to care plans as needed
• Maintain accurate records and documentation of client care and services provided
• Collaborate with the healthcare team to ensure seamless transitions and continuity of care

• Minimum of 2 years of experience in home health care or a related field
