Advanced Care Partners is seeking a Registered Nurse (RN) to provide individualized care for medically fragile children and adults in home and school settings. This position offers a more personalized care model, allowing for stronger patient outcomes and professional satisfaction.

• Base Pay: $45.00 hourly
• Differentials:
  • Trach: +$2.00 per hour
  • Vent: +$3.00 per hour
  • Nights: +$2.00 per hour
  • Weekends: +$3.00 per hour
  • Holidays: +$3.00 per hour
• Flexible scheduling (full‑time, part‑time, PRN)
• Cases located near your preferred work area
• Consistent patient assignments

• Provide skilled nursing care, including feeding tubes, tracheostomies, ventilators, and central lines
• Monitor patient progress and respond to changes in condition
• Educate patients and families on treatment plans
• Maintain a safe and clean care environment

• Active RN license in Georgia or participation in the Nurse Licensure Compact
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization
• Minimum of one year of RN experience
• Valid PPD skin test

• Ability to stand or sit for extended periods (up to 12 hours)
• Ability to bend, lift, and assist with patient mobility
• Ability to lift up to 50 pounds or more
• Fine motor skills for clinical tasks
