• Conducts/participates in quarterly quality improvement meetings, reviews data, identifies trends, and oversees implementation of improvement plans.
• Collaborates with the Branch Director to implement quality improvement activities and corrective action plans.
• Oversees response to external and internal surveys and audits regarding quality of care and KPI performance.
• Completes documentation and chart reviews to identify variances and improve performance.
• Holds case conferences to ensure oversight of care and coordination of services.
• Facilitates development of performance improvement action plans, communicates job expectations, plans, monitors, and evaluates employee performance.
• Ensures staff are evaluated fairly and timely in accordance with company procedures.

• Bachelor’s Degree preferred in Nursing from an accredited school of nursing.
• Licensed RN in good standing and currently licensed in the state of practice and residence.
• Less than two years of clinical experience, and at least one year in a related management/supervisory role in home health care.
• Current CPR certification.
• Must meet all agency pre‑employment requirements.
• Ability to use company documentation systems.
• Strong verbal and written communication skills at all levels.
• Can work independently as well as part of a team.
• Capable of handling confidential information responsibly.
