Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner (PRN) to join our Healthcare & Medical Services team. In this flexible, as‑needed role, you will perform comprehensive in‑home health assessments and targeted clinical services for health plan members, helping to improve patient outcomes and reduce overall healthcare costs.
As a Nurse Practitioner (PRN) with Matrix Medical Network, you will travel to members’ homes or designated community locations to complete detailed histories and physical examinations, chronic disease evaluations, medication reconciliations, and preventive health screenings. You will synthesize clinical findings, identify care gaps and risks, and document accurate, concise assessment reports using electronic health record systems. Your work will support health plans, government agencies, and primary care providers by giving them a clearer picture of each member’s health status.
You will collaborate closely with an interdisciplinary team, including physicians, clinical leadership, quality assurance staff, care coordinators, and health plan partners. Drawing on your advanced clinical skills, you will provide patient education on disease management, medications, lifestyle changes, and appropriate follow‑up care, tailoring your approach to each member’s needs, literacy level, and cultural context. When clinically indicated, you will recommend follow‑up with primary or specialty providers, community resources, or additional diagnostics, helping to close quality and care‑coordination gaps.
This role is ideal for a Nurse Practitioner who enjoys autonomy in the field, varied patient encounters, and the opportunity to focus on prevention and quality rather than volume‑based clinic visits. You will manage your schedule within PRN parameters, adapting to changing caseloads and geographic assignments, while maintaining high standards for clinical accuracy, timeliness of documentation, and adherence to all regulatory, payer, and organizational policies.

A master’s degree or higher in nursing, active state NP and RN licensure, national board certification, and current BLS are expected. Familiarity with Medicare, Medicaid, and managed‑care populations, along with comfort using laptops or tablets and electronic health records, will support your success in this role.
