Description
JOB SUMMARY:Assists with coordinating treatment to provide comprehensive wound assessments. Follow plan of care. Provide scheduled wound treatments as ordered by physician. Assists with patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. Assists with performance improvement plan of the organization.
EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:- Graduate of state approved school of practical nursing and has passed the state board examination.
- Current state License Practical Nurse Licensure.
- Minimum of two (2) years' experience in long term care nursing.
- Current BCLS Certification
- Performs all job responsibilities in alignment with the mission and vision of the organization.
- Performs other duties as required and completes all job functions as per departmental policies and procedures.
- Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs).
- Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.
- Wears protective clothing and equipment as appropriate.
SKILLS:
- Ability to communicate in English, both verbally and in writing.
- Additional languages preferred.
- Strong written and verbal skills.
- Basic Computer Skills
- General environment:
Works in a well-lighted, air-conditioned area, with moderate noise levels. - May be exposed to high noise levels and bright lights.
- May be exposed to limited hazardous substances or body fluids, or infectious organisms.
- May be required to change from one task to another or different nature without loss of efficiency or composure.
- Periods of high stress and fluctuating workloads may occur.
- May be scheduled as needed including overtime.
- Have near normal hearing:
Hear alarms/telephone/normal speaking voice. - Have near normal vision:
Clarity of vision (both near and far), ability to distinguish colors. - Have good manual dexterity.
- Have good eye-hand foot coordination.
- Ability to perform repetitive tasks/motion.
- Continuously within shift (67-100%):
Standing, Walking. - Frequently within shift (34-66%):
Bending/Stooping, Pushing/Pulling, Lift/carry up to 20lbs, Lift/carry greater than 20 lbs. with assistance. - Occasionally within shift (1-33%):
Sitting, Climbing, Twist at waist, Lift/Carry greater than 50 lbs. with assistance, Reaching above shoulder.

JOB SPECIFICCOMPETENCIES:
- Ability to perform wound assessment, reassessment and management of all patients and per policy. This includes geriatric and the general population.
- Knowledgeable of the anatomy and physiology and disorders of the gastrointestinal, genitourinary and dermal systems.
- Utilizes principles of current wound management theory when providing care to patient with acute and chronic wounds.
- Demonstrates thorough knowledge of staging system of wounds.
- Follows skin care programs to prevent skin breakdown of patient.
- Ability to follow plan of care.
- Ability to adequately assess and reassess pain.
- Demonstrates ability to perform treatments and provide services to level licensure.
- Demonstrates ability to assist with a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
- Knowledgeable of medications and the correct administration based on age of the patient and clinical condition.
- Follows the five (5) medication rights and reduces the potential for medication errors.
- Identifies and addresses psychosocial needs of patients and families; communicates with social service/discharge planner as appropriate.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
