WhatYou’ll Do
- Performing nursing assessments and obtaining vital signs, health histories, and other clinical information
- Performing routine tests and screenings and communicating findings or concerns to providers
- Administering medications, treatments, injections, and immunizations as ordered
- Assisting providers with patient examinations and procedures
- Triaging patient calls and visits within the LPN scope of practice
- Providing patients and caregivers with education regarding medications, treatment plans, preventive care, and other healthcare needs
- Coordinating specialist referrals, testing, procedures, appointments, and prior authorizations
- Supporting patients with complex healthcare needs through care coordination and care management activities
- Maintaining accurate and complete clinical documentation
- Reviewing immunization status and administering and documenting immunizations
- Collaborating with providers, clinical staff, patients, caregivers, and outside healthcare facilities to support continuity of care
- Participating in Quality Improvement activities and supporting CCHS clinical standards and protocols
- Degree or diploma in Practical Nursing from an accredited nursing program
- Current and valid Licensed Practical Nurse (LPN) license in the state of Georgia
- Current BLS/CPR certification
- One to three years of nursing experience in an outpatient or primary care setting preferred
- Knowledge of nursing assessment, medication administration, injections, immunizations, and other clinical procedures within the LPN scope of practice
- Strong communication, organization, and patient education skills
- Ability to recognize and communicate changes in a patient's condition and respond appropriately
- Ability to use an Electronic Health Record (EHR) system
- Ability to work collaboratively as part of a healthcare team
- Bilingual in English and Spanish is a plus
Hours:
Monday - Friday: 8:00am - 5:00pm
