The Registered Nurse (RN) is responsible for providing direct nursing care to residents in accordance with physician orders, facility policies, and standards of professional nursing practice. The RN plays a vital role in ensuring the health, safety, and well-being of residents by delivering high-quality, resident-centered care in a skilled nursing setting. This position includes supervising Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs), participating in care planning, and collaborating with the interdisciplinary care team to promote optimal resident outcomes.

• Assess, plan, implement, and evaluate nursing care based on residents' individual needs, conditions, and goals.
• Administer medications and treatments as prescribed, following the "five rights" of medication administration and facility policies.
• Monitor residents' conditions and promptly report changes to physicians, nurse practitioners, and appropriate members of the care team.
• Perform clinical procedures such as wound care, catheter care, intravenous (IV) therapy, medication administration, and other skilled nursing interventions within the RN scope of practice.
• Respond to emergencies and significant changes in resident condition in a timely, professional, and effective manner.

• Complete nursing documentation accurately, thoroughly, and in a timely manner, including assessments, progress notes, care summaries, and nursing observations.
• Maintain and update individualized care plans in collaboration with the interdisciplinary team.
• Ensure accurate and complete Medication Administration Records (MARs) and Treatment Administration Records (TARs).
• Document incidents, resident concerns, physician notifications, and changes in condition according to facility policy and regulatory requirements.

• Supervise LPNs and CNAs by providing clinical guidance, support, delegation, and direction.
• Delegate tasks appropriately based on staff competency, scope of practice, resident acuity, and facility needs.
• Monitor assigned staff performance to ensure compliance with nursing standards, care plans, regulatory requirements, and facility policies.
• Participate in orientation, training, competency validation, mentoring, and performance feedback activities as assigned.

• Communicate resident care plans, progress, treatment outcomes, and changes in condition to residents, families, and responsible parties as appropriate.
• Respond promptly, professionally, and compassionately to resident and family concerns.
• Support residents' emotional, physical, psychosocial, and spiritual needs while promoting dignity, respect, and independence.

• Follow infection prevention and control practices, safety protocols, emergency preparedness procedures, and resident rights policies at all times.
• Maintain confidentiality and protect resident information in accordance with HIPAA and facility standards.
• Adhere to all federal, state, and local regulations governing nursing practice and long-term care operations.
• Participate in audits, Quality Assurance and Performance Improvement (QAPI) activities, risk management initiatives, and emergency preparedness drills.

• Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification preferred.
• Regular and reliable attendance is required to perform the essential functions of this position.
• Current, active Registered Nurse (RN) license in the state of employment required.
• Graduate of an accredited school of nursing;
  Associate Degree in Nursing (ADN) required, Bachelor of Science in Nursing (BSN) preferred.
• Previous nursing experience in skilled nursing, rehabilitation, long-term care, post-acute care, or acute care settings preferred.
• Experience supervising, mentoring, or leading nursing staff preferred but not required.
• Strong clinical assessment, critical-thinking, and problem-solving skills.
• Effective communication, leadership, and interpersonal abilities.
• Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
• Ability to prioritize responsibilities and manage time effectively in a fast-paced healthcare environment.
• Basic computer skills and familiarity with electronic health records (EHR) and clinical documentation systems.
