Position Overview

The Life Skills Coach (LSC) is a community‑based position working directly with individuals who have sustained a brain injury or have some other neurological condition. The LSC provides functional supports to clients on an individualized basis designed to assist them in acquiring, retaining and improving the skills necessary to reside independently at home and access a variety of community‑based resources.

Basic

Job Duties
  • Provides positive support and advocacy for the client.
  • Assists the client with identifying choices and making informed decisions regarding activities and life.
  • Provides structure and support in household management, such as maintaining cleanliness and establishing a safe, healthy and efficient routine.
  • Assists with participation in recreation and community activities to promote socialization and relationships with peers.
  • Improves community living and integration skills in settings such as shopping, church attendance, sporting events, club participation, classroom support, volunteer or employment support, and provides transportation as needed.
  • Teaches the client to participate in recreational and social activities within the surrounding community.
  • Provides training or assistance with mobility in the home environment, using adaptive aids, and accessing public transportation.
  • Assists with developing effective communication and social skills.
  • Operates within organizational guidelines, policies, professional and ethical standards.
  • Maintains confidentiality of client information in accordance with HIPMA/company policies.
  • Treats clients, families, co‑workers and outside parties with respect and dignity, honoring all organizational values and ethical practices.
  • Provides functional supports to acquire, retain and improve independence based on established goals.
  • Helps clients reintegrate and reconnect with their communities to encourage a healthy level of independence.
  • Identifies barriers, triggers or changes in functional status to develop proactive interventions that promote continued independence.
  • Educates clients on developing problem‑solving skills and strategies.
  • Accompanies clients to locations and appointments, teaching them to assess and utilize community resources and services.
  • Serves as a role model and mentor, maintaining good communication and relationships with the client, families and referral sources.
  • Completes daily progress notes and other necessary documentation in compliance with organizational policies and submits them to supervisors and the agency’s payroll/invoicing portal.
  • Monitors and appropriately utilizes billable hours with clients.
  • Conducts professional behavior at all times.
  • Participates in agency trainings and other relevant training.
  • Communicates regularly with organization personnel regarding issues and concerns.
  • Participates in team meetings as requested.
  • Performs other duties as assigned to meet program goals or as requested by the agency.
Qualifications
  • Experience with individuals with developmental disabilities and in some cases physical disabilities preferred.
  • Must have a reliable computer or device for documentation responsibilities; experience with Microsoft Word, Excel and Adobe Acrobat Reader required.
  • BS/BA in a related field or 1–2 years of experience working with adults who are developmentally disabled or other related experience preferred.
  • Maintain a valid driver’s license.
  • Background check and driver’s record check.
  • Reside within a reasonable commute to the client.
  • Must maintain the company’s required personal auto insurance and verify compliance: minimum coverage of $100,000/$300,000/$50,000 (bodily injury/bodily injury per accident/property damage) or $300,000 combined single limit.
  • Military background or experience with veterans and persons with brain injury a plus.
  • The ability to lift 25/50/80 pounds regularly is required.
  • Strong written and verbal communication skills.
  • Strong interpersonal skills and ability to collaborate with team members.
