Ennoble Care is seeking an experienced, per diem Hospice Social Worker to service patients in our Savannah, GA region and surrounding areas.
Responsibilities- Conduct a psychosocial assessment within 5 days of admission to the hospice program, identifying emotional, social, financial, environmental resources and appropriate psychosocial problems, interventions and goals for the patient’s plan of care.
- Participate in IDG meetings and in developing the patient’s plan of care.
- Observe, record, and report changes in the patient’s emotional and social factors affected by the illness and care response.
- Maintain and submit written clinical records (initial evaluation, care plan, daily notes) as required.
- Evaluate the patients and family's response to and effectiveness of the medical social work intervention.
- Confirm, on a weekly basis, the scheduling of visits with the patient’s Case Managers to coordinate other staff visits.
- Participate in IDG and QAPI meetings and activities.
- Continue to offer support to the IDG if the patient/family declines Social Work services and monitor evolving needs.
- Assume responsibility for self‑development by pursuing formal education, workshops, conferences, professional meetings, and research.
- Assess the patient’s or family members’ ability to cope with the patient’s dying.
- Act as consultant to health team members, assisting them in understanding social, emotional, and environmental factors related to the patient’s health problems.
- Help patients utilize resources of their families and the community.
- Licensed by the GA State Board of Social Work.
- Master’s degree in social work from a Council on Social Work Education–accredited graduate school.
- Licensed Master Social Worker (LMSW).
- At least one year of post‑master’s social work experience in a health care setting.
- Hospice experience preferred.
- Ability to reliably handle and maintain confidential information.
- Licensed driver with an insured automobile in good working order.
- Excellent verbal and written communication skills.
Pay Rate: $50.00 per visit, $85.00 per admission, and mileage reimbursement at $0.65 per mile.
