Job Summary:

Seeking a licensed Physical Therapist to provide patient care across a variety of clinical settings. The therapist will be responsible for implementing treatment plans, documenting patient progress, and ensuring quality care delivery.

Responsibilities:

• Evaluate patients to determine their individual needs and develop personalized treatment plans.
• Implement and adjust therapy interventions based on patient progress and clinical assessments.
• Document all patient interactions, treatment progress, and clinical notes accurately and in timely manner.
• Educate patients and caregivers on treatment techniques, exercises, and home programs.
• Collaborate with healthcare team members to ensure coordinated patient care.
• Adhere to all licensing, certification, and equipment protocols required for safe practice.

Qualifications:

• Active state license as a Physical Therapist.
• Proven experience in clinical settings with documentation skills.
• Ability to evaluate and develop treatment plans for diverse patient populations.
• Strong communication and patient education skills.
• Knowledge of current therapy techniques and equipment.
• Reliable transportation and flexibility to work assigned shifts.
