LVN ​/ LPN – LTC​/SNF

Nursing Care Provider

Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting during overnight shifts. Ensure patient safety, comfort, and compliance with healthcare standards.

Responsibilities:

• Administer medications and treatments according to physician orders.
• Monitor residents’ vital signs and overall health status throughout the shift.
• Document patient care activities accurately and thoroughly.
• Collaborate with healthcare team members to develop and update care plans.
• Respond promptly to emergency situations and implement appropriate interventions.
• Maintain a safe, clean, and organized environment for residents and staff.

Qualifications:

• Valid LVN or LPN license in good standing.
• Minimum of 1-year experience in LTC or SNF environments preferred.
• Ability to work overnight shifts and rotating weekends.
• Must bring a valid state or federal  wear a photo badge at all times.
• Be vaccinated against COVID-19 or have an approved medical/religious exemption.
• Strong communication skills and attention to detail.
