Home Care Certified Nursing Assistant

Encore Village is currently hiring for PRN (as needed) Home Care Certified Nursing Assistants. The Home Care C.N.A. provides personal care and supportive services that are intended to assist clients in accordance with individualized service and nursing treatment plans, which promotes their physical and emotional wellbeing and encourages them to reach their highest level of independence.

• Provides personal care to Home Care Residents
• Recognizes an emergency situation and acts accordingly.
• Completes necessary documentation associated with visits.
• Maintains current knowledge in area of specialty and activities within agency.
• Performs other duties as assigned.
• Assists in teaching healthcare procedures to residents and/or caretaker including safety measures.
• Documents each home visit including acre rendered in EHR.
• Submits documentation to office according to established time frames.
• Participates in orientation of new home care staff as to the role and duties of Home Care Aides.
• Reports any changes or emergencies to supervisor immediately.
• Maintains confidentiality of all residnets.
• Attends scheduled staff meetings and in-services as appropriate.
• Provides feedback to supervisors regarding resident's ability to care for needs in apartment.
• Work as a team member to assure residents receive timely, competent, compassionate and respectful care.
• Answers and directs phone calls to the agenct appropriately.

• Successful completion of basic course of instruction for C.N.A. which has been approved by IDPH
• Current CPR Card
