RN Cart Nurse

Schenectady Center seeks a compassionate RN Cart Nurse to provide resident-focused skilled nursing care on our long-term and rehab units. The RN will manage medication and treatment carts, complete assessments, implement and update care plans, and coordinate with an interdisciplinary team to support safe, high-quality outcomes. Responsibilities include supervising CNAs, monitoring changes in condition, documenting in the EMR, and communicating with providers and families.

The role offers ongoing clinical training, collaboration, and professional growth in a supportive, resident-centered environment serving elderly and rehabilitative patients.

Responsibilities

• Administer medications and treatments from the cart per provider orders and facility protocols
• Perform ongoing resident assessments and document findings in the EMRDevelop, implement, and update individualized care plans for rehab and long-term care residents
• Monitor for changes in condition and promptly notify providers and families
• Supervise, support, and delegate tasks to CNAs and other support staff
• Ensure infection control and resident safety standards are consistently followed
• Coordinate with the interdisciplinary team on discharge planning and clinical goals
• Educate residents and families on care plans, medications, and health management
• Participate in audits, quality improvement, and regulatory compliance activities
• Assist with orientation and clinical training of new nursing staff as needed
