Registered Nurse Case Manager

Methodist Hospital, part of SA Health, is seeking an experienced Registered Nurse Case Manager to join our Healthcare & Medical Services team in San Antonio. In this vital role, you will blend direct patient interaction with coordinated care planning to ensure safe, efficient, and compassionate transitions of care for a diverse patient population.

As a Registered Nurse Case Manager, you will perform comprehensive nursing and psychosocial assessments, identify clinical and discharge needs, and develop individualized, evidence‑based care plans. You will coordinate services throughout the continuum of care, from admission through discharge and follow-up, with a strong focus on reducing avoidable readmissions, optimizing length of stay, and supporting high‑quality outcomes. Your responsibilities will include facilitating communication among providers, arranging post‑acute services such as home health, rehabilitation, and community resources, and educating patients and families on diagnoses, medications, treatment options, and self‑management strategies.

You will leverage strong clinical judgment, critical thinking, and knowledge of utilization management to ensure that patients receive appropriate levels of care, while also navigating insurance requirements and regulatory guidelines. Using the hospital’s electronic health record, you will document assessments, interventions, and outcomes accurately and in a timely manner.

You will contribute to ongoing performance improvement efforts, participate in multidisciplinary rounds, and support hospital initiatives that advance patient safety, quality, and patient experience.
