In Home Healthcare LVN - Acuity; Weekend

Join a patient-centered healthcare team as a Licensed Practical / Vocational Nurse (LPN/LVN).

• Providing skilled nursing care under supervision, following care plans and physician orders.
• Assessing patient conditions, documenting activities, and updating care plans.
• Monitoring for changes in patient status and implementing appropriate interventions.
• Educating patients and families on disease management, self-care, and prevention.
• Ensuring infection control and compliance with regulations and standards.

• Active, unrestricted LPN/LVN license in the practicing state.
• Current CPR certification and TB test within the past 12 months.
• Preferred: six months of clinical nursing experience, home health experience, and proficiency in assessments and documentation.
• Must have reliable transportation and physical ability to lift and reposition patients.
