LVN Intake Nurse WellMed at Stone Oak

The Licensed Vocational/Practical Nurse (LVN/LPN) provides clinical support and nursing care under the supervision of providers and RNs. Responsibilities include performing procedures such as injections, medication administration, EKGs, and phlebotomy, assisting with patient education, coordinating care, documenting assessments, and ensuring a safe, confidential environment in compliance with standards and policies. The role involves preparing patients, collecting data, relaying information to providers, managing medications, and maintaining infection control and documentation accuracy.

Certification in IV therapy is preferred.

• High school diploma or GED
• Current LVN/LPN license and BLS certification
• Knowledge of medical terminology and procedures
• Proficiency with electronic medical records
• Effective communication, customer service, and emergency response skills
• Ability to perform medical procedures, including injections, phlebotomy, and IV therapy (if certified)
• Ability to establish effective relationships with staff and patients
• Physical ability to handle equipment and perform duties safely

• Perform nursing procedures within scope of practice
• Implement patient education plans and coordinate care
• Document patient assessments and care accurately
• Collect lab samples, manage medications, and ensure infection control
• Assist with patient intake, preparation, and follow-up care
• Maintain a safe and respectful environment following standards and policies
