Position Summary
  • The Education & Case coordinator delivers individual and group education sessions, provides client-centered case management, and builds strong community partnerships to support fathers in overcoming barriers and strengthening family relationships. Services are delivered in a variety of settings, including community locations, client homes, and correctional facilities.
Job Duties
  • Provide direct case management services to clients in community settings including client homes, jails, and other designated locations.
  • Conduct participant intake and assessment; develop Individual Service Plans (ISPs).
  • Deliver curriculum-based education and training sessions to individuals and groups, ensuring fidelity to approved program models.
  • Facilitate client engagement, peer‑to‑peer participation, and group activities.
  • Provide referrals and linkage to community resources, including food assistance, education, employment, housing, mental health, and other supportive services.
  • Assist clients with applications for public and private benefits as needed.
  • Coordinate and provide transportation for program‑related appointments, as needed.
  • Maintain accurate client documentation, attendance records, assessments, and outcome data in designated system(s).
  • Track program outcomes and support data collection and reporting requirements in collaboration with program leadership.
  • Conduct outreach, networking, and relationship‑building activities to promote the program and generate referrals.
  • Attend and coordinate internal meetings, partner meetings, and community events to sustain partnerships, increase program visibility, and support service delivery.
  • Develop and maintain collaborative relationships with community partners, including nonprofit agencies, correctional facilities, employers, healthcare providers, and social service organizations.
  • Maintain compliance with confidentiality, PHI/PII requirements, and all applicable program and funding requirements, regulations, and best practices.
  • Participate in curriculum updates, training, and continuous program improvement efforts.
  • Perform other related duties as assigned.
Required Skills/Abilities
  • Excellent verbal and written communication skills, with the ability to build rapport and maintain professional boundaries with clients and community partners.
  • Able to deliver education and training to diverse populations in various settings.
  • Able to balance education delivery, case management, documentation, and outreach responsibilities effectively.
  • Strong organizational skills with attention to detail and ability to meet deadlines.
  • Demonstrated integrity, professionalism, and commitment to confidentiality.
  • Able to pass required background checks and be cleared to work in correctional settings.
  • Able to work independently, manage a variable schedule across multiple service locations, and adjust work priorities based on supervisory direction and feedback.
  • Awareness and sensitivity of the constituents and the populations served by employees.
  • Proficient with Microsoft Office Suite or related software.
  • Regular and predictable attendance, and promptness for work.

Education and Experience
  • Bachelor’s degree in human services, social work, education, public health, criminal justice, or related field is preferred. Relevant experience may be considered in lieu of a degree.
  • One or more years’ experience providing training, facilitation, or curriculum‑based education.
  • One or more years’ experience providing case management or direct client service.
  • Valid driver’s license, proof of liability automobile insurance, and the ability to drive a personal vehicle for agency business.
Physical Requirements
  • Hybrid work environment with in‑office and regular community‑based work.
  • Able to travel locally and regionally to meet program needs, including community‑based work, to and from correctional facilities, and occasional training or conference attendance.
  • Able to work a flexible schedule, including occasional evenings and/or weekends.
  • Able to use standard office equipment, including company‑issued laptop and cell phone.
